Registration You must register online for the 2004 SURA/ViDe Workshop through the Acteva website. (You may be prompted to create a MyActeva account. It is not necessary to have a MyActeva account to register for the workshop. Simply select the "Continue without Logging In" option.) Early registration is $175 per person, which includes receptions Monday and Tuesday evening, and continental breakfasts, morning and afternoon breaks and bistro lunch Tuesday and Wednesday. Payment may be made by Mastercard, VISA, American Express, Discover, check or purchase order. The fee for those registering after February 22 is $225. The final day for registration is March 12. Hotel The 2004 SURA/ViDe Workshop will be held at University Place Conference Center and Hotel, Indiana University Purdue University Indianapolis (IUPUI), in Indianapolis, Indiana. For reservations, please call (317) 269-9000 or toll-free (800) 627-2700. A limited number of rooms have been reserved for conference attendees. Rates for conference attendees are $122 (plus tax) per night for a single room, $137 (plus tax) per night for a double room. These rates are available only until March 1. Parking is available at a rate of $6 per day. Transportation Attendees are responsible for their own transportation while in Indianapolis. Round-trip shuttle service is available from Indianapolis International Airport to University Place (approximately 20 minutes each way). Shared ride shuttle services are provided by these three companies: Affinity Limousine & Tours, Inc. (317-870-5466), Carey Indiana (800-888-4639) and Hoosier Limousine, Inc. (317-248-8879/0885).
