3. Enter the name of your organization (university, business, etc.).
4. Enter a title that describes your work (multimedia specialist, educator, etc.).
5. Enter the first portion of your address.
6. Enter additional address information, if any.
7. Enter the city in which you work.
8. Enter the state or province in which you work.
11. Enter your phone number with applicable area code, country code, etc.
13. Enter the type of endpoint you are using (VCON VIGO, PolyCom ViewStation, etc.).
14. Please provide a title for your endpoint information, such as: Desktop Conferencing Endpoint or Conference Room Endpoint.
15. Please provide a name which will be used in your H.323 Alias and H.323 URL. It is suggested that you use your login or username. No spaces are allowed
16. Check this circle in the event that you do NOT wish to be listed in a directory.
17. Please select the zone that you wish to be listed in. In the event that you do not select a zone, you will be placed in the ViDeNet public Zone.
18. Make sure that all required fields have been carefully entered and submit. .
19. This field is available in the event that the zone administrator wishes to assign a number to the user for tracking, billing, etc.
20. This field allows the zone administrator to see when the entry was created.
21. This field allows the zone administrator to see when the entry was last modified..
22. Selected if this directory entry is active.
23. Selected if this directory entry is pending approval.
